Core Objectives:

Why It Matters:

Far too often, individuals with developmental disabilities face fractured systems, housing barriers, and limited access to coordinated supports. The Stabilities Pathways Project bridges these gaps — working alongside individuals to build sustainable foundations for their future, while also advocating for system-level change.

Whether someone is transitioning from family care, exiting the child welfare system, or recovering from a period of instability, this project provides a lifeline — offering the tools, partnerships, and encouragement needed to move forward with dignity and purpose.

The Stabilities Pathways Project is a specialized initiative developed by Rise & Thrive Support Services to address the critical need for long-term stability among individuals with developmental disabilities, complex support needs, addictions and mental health. Rooted in principles of person-directed planning, equity, and community inclusion, the project offers a structured and compassionate approach to navigating life’s essential transitions — including housing, health, employment, and social connection.

This project is designed for individuals who are at risk of housing insecurity, isolation, or systemic barriers that limit their access to safe, supported, and meaningful living. Through individualized service navigation, skill-building workshops, and community engagement, the Stabilities Pathways Project helps people create achievable, goal-oriented plans while building confidence and resilience.

Continuity of Services: Enhancing Outcomes Through Consistent, Coordinated Support

At Rise & Thrive Support Services, we recognize that continuity of services is a critical factor in achieving positive, sustainable outcomes for individuals with developmental disabilities and complex needs. Disruptions in care, changes in providers, or inconsistent access to supports can negatively impact progress, increase risk, and reduce trust in service systems.

That’s why our model prioritizes consistent, person-centered support across all service streams, including housing navigation, Adult Protective Services, skill development, respite, and community inclusion. By maintaining reliable relationships, shared planning, and coordinated transitions, we create a stable foundation where individuals can grow with confidence and clarity.

Our Clients

Rise & Thrive Support Services recognizes the urgent need for housing and supports in our own community of Northumberland County. We are committed to serving those in our community who need us most. Clients can be self-referred, agency referred, or family/friend referred.

Our Approach to Continuity:

  • Long-Term Service Planning: We support clients through multiple life stages and transitions, offering guidance that adapts as their goals evolve.

  • Integrated Case Management: Staff collaborate across programs and external partners to ensure seamless coordination and reduce service fragmentation.

  • Relational Consistency: Whenever possible, clients are supported by the same trusted staff members, fostering connection, safety, and trust.

  • Clear Communication & Documentation: Continuity is reinforced through transparent care plans, regular progress reviews, and shared information (with consent).

  • Proactive Transition Supports: We guide individuals through planned and unplanned transitions — such as moving, hospitalization, or aging out of other services — with stability and care.

Continuity reduces barriers, improves engagement, and enhances program effectiveness. Clients experience fewer setbacks, stronger outcomes, and a deeper sense of belonging and empowerment. For families and partners, continuity builds confidence that services are dependable, respectful, and forward-thinking.
