Q: What is Rise & Thrive Support Services?
A: We are a community-based agency offering developmental services, mental health supports, housing stability programs, and individualized services for children, youth, and adults. Our goal is to empower individuals through person-centered support that promotes independence, inclusion, and well-being.
Q: Where are you located?
A: We are based in Northumberland County, Ontario, with programs operating in various surrounding regions. Our services include both in-person and mobile supports, depending on the program. Our main office is located at 304 Division Street, Cobourg.
Q: Who can access your services?
A: We support individuals of all ages with developmental disabilities, mental health needs, housing instability, and transitional support needs. Services are available through direct referrals, fee-for-service, or funding-supported pathways such as MCCSS, Passport, ODSP, or OAP.

Q: How do I know which service is right for me or my loved one?
A: We provide a free intake consultation to explore your needs and goals. Based on this, we guide you to the most appropriate service stream.
Q: Do you offer services for people experiencing homelessness or housing instability?
A: Yes! Our Thrive Housing & Stability branch supports individuals with housing navigation, crisis outreach, transitional housing, and mental health support. -
Q: How do I make a referral?
A: You can submit a referral by downloading our referral form or email us at intake@riseandthrivesupportservices.com. We accept referrals from families, individuals, agencies, and caseworkers.
Q: Is there a waitlist?
A: Some services may have limited spots depending on capacity. We will always do our best to provide timely support or offer alternate service options.
Q: Can I refer someone in crisis?
A: Yes — our Stability Pathways Project allows immediate crisis response and intake for individuals in urgent need of housing or mental health support.

Q: Do you accept Passport funding?
A: Yes. Many of our services are eligible for Passport or DSO-supported funding. We also offer private fee-for-service options.
