A Program Focused on Filling the Gap to Adulthood

New Beginnings is a transitional residential care model supporting Transitional Aged Youth (TAY), ages 14 and older, as they prepare for adulthood. The program is designed to address the critical gaps that often emerge between traditional child welfare services and independent adult living, offering a continuum of care from intake and assessment to fully independent living with continuity supports.

  • • Provide a stable and nurturing living environment for youth in transition.

    • Foster development of independent living skills including budgeting, self-care, employment readiness, and tenancy maintenance.

    • Support youth in achieving educational, vocational, and personal development goals.

    • Deliver trauma-informed, culturally responsive, and person-centered care.

    • Coordinate access to mental health, medical, and behavioural supports as needed.

    • Facilitate gradual transition from high-support to independent living environments.

    • Promote long-term stability, community integration, and positive social outcomes.

    • New Beginnings Home Base (Intake & Assessment Home)

    This is the first point of entry for all participants. Youth are welcomed into a supportive, structured group living environment where initial assessments, planning, and goal-setting are completed by a multidisciplinary team. Each individual receives a customized care plan that informs their pathway through the broader continuum of services.

    • TAY Intensive Support and Complex Care Homes

    Designed for individuals identified through assessment as requiring higher levels of support due to complex needs. These group homes offer 24/7 staffing with specialized training in behavioural, clinical, and trauma-informed care. Support is centered on therapeutic engagement, daily living skill development, and progressive readiness for more independent living environments.

    • Supported Independent Living (SIL)

    SIL supports are offered to individuals who demonstrate the skills and stability required to live semi-independently. Participants reside in self-contained market-rent units, often clustered in a building with on-site staff offices. Support is scheduled based on individualized service agreements and may include assistance with medical appointments, mental health, education, or employment.

    • Complex Care Supported Independent Living (CCSIL)

    This model provides a higher-support version of SIL for individuals with complex clinical or behavioural needs. Participants live independently but with 24/7 staffing supports at entry. Over time, the level of support is gradually reduced as individuals demonstrate readiness, guided by their clinical team and care plan.

    • Continuity Case Management (CCM)

    For individuals who have transitioned into independent living, Rise & Thrive offers optional ongoing case management. CCM ensures continuity of care, links individuals to community resources, and provides flexible, responsive support that promotes long-term success, safety, and stability in the community.

    • Target Population

    Youth aged 14 and older with a focus on transitional-aged youth preparing to exit care, including those with complex needs requiring a high degree of clinical or behavioural support. Referrals may include individuals involved with child welfare, youth justice, or developmental services systems.
