New Beginnings

A residential treatment and assessment home for transitional aged youth meant as a stepping stone to independence.

Referral:

This process can start as early as 14 years of age to help prepare youth and caregivers for integration into Rise & Thrive’s New Beginnings home.

Rise & Thrive is currently accepting referrals for our New Beginnings Program!

Integration:

A specialized TAY coordinator will ensure that the youth and their support network are fully informed of the process for integration into Rise & Thrive. From there, the TAY coordinator is responsible for developing an individualized transition plan for the youth into the New Beginnings home. This transition plan will identify the responsibilities of individuals, families, and service providers for the successful integration to New Beginnings and onward from youth to adult services. The TAY coordinator will remain involved in each Individual's journey until they transition out of New Beginnings. 

Once an individual transitions to Rise & Thrive and into New Beginnings, the Clinical Support Team will facilitate their individual plan in conjunction with a diverse staff team consisting of highly trained Child and Youth Workers, Developmental Support Workers, and Social Service Workers.

Intensive Support for Complex Care

Individuals who have been assessed in our base homes and have been identified as needing a more complex support base will be placed in these residential settings

. These homes have 24/7 staff who are highly trained and qualified to manage complex special needs and behaviours.

Specialized care plans are developed with each Individual and their clinical support team. This is a group setting, with Individual goals centered on living as independently as possible, which could mean transitioning to one of our CCSIL units.

Complex Care Supported Independent Living

Similar to our SIL programs, each Individual will live on their own in a self-contained unit. Each Individual that moves to this program has been assessed by their clinical support team for suitability.

Within the CCSIL units, each individual still has 24/7 staffing to begin with. The individual will then work on their goals through the care planning with the clinical team to achieve further independence.

There is always a goal to reach, and progress to make. Transitioning from CCSIL to SIL is always possible.
