Welcome to Rise & Thrive Support Services — where support empowers and services uplift. Established in 2022.
Our mission: to help individuals with developmental disabilities and complex needs live fulfilling, self-directed lives in their communities.
We offer a range of person-centered services across Ontario, including:

Adult Protective Services (APS)
Case Management & Service Navigation
Respite & Passport Program Support
Person-Directed Planning
